If you would like to become a client of Terra Nova Midwifery please submit the Care Request Form (below) as soon as possible. Space is limited and fills quickly! Before submitting the form please ensure you have entered all information correctly.
You will be contacted within 1-3 days to be advised whether or not there is space available for your estimated due date. If you would like to be placed on a waitlist if space is not currently available, please select the waitlist box in the form. We will contact you if a space becomes available.

Please note that this Request Form is for FULL Midwifery Care (pregnancy, birth and postpartum). If you wish to be in Postpartum-Only Care, fill out the Postpartum-Only Care Request Form here.
